For Providers

For referring providers and partners

Grava Health is a telehealth behavioral health practice serving adults located in Colorado. We provide individual therapy, family support, outpatient group therapy, and substance use counseling for co-occurring presentations. Non-clinical life coaching is available separately on a cash-pay basis; it is not therapy and does not diagnose or treat.

Who fits well here

  • Adults stepping down from residential, PHP, or IOP who need outpatient continuity and a plan for the transition home.
  • Clients with substance use concerns at any stage, from pre-contemplation to sustained remission, with or without a formal diagnosis.
  • Presentations rooted in relational and developmental trauma, low self-worth, perfectionism, codependency, and difficulty with life-stage transitions.
  • Family members of someone in treatment, including families with no identified client currently in care and those managing a relationship with a loved one in active use.
  • Professionals and former athletes whose long-standing coping strategies have become maladaptive, often presenting as burnout, anxiety, or substance use.
  • Clients who would benefit from integrated care beyond weekly psychotherapy alone, where clinical work is coordinated with non-clinical health and performance coaching.

We are strictly an outpatient practice. Our main disqualifiers include the need for withdrawal management or acute stabilization, active SI/HI/SH, psychosis, an active eating disorder, recent violence or arson, and any presentation requiring 24-hour care.

How to refer
